Patient Safety Solutions: Alerts & Notices

Following the abolition of the National Patient Safety Agency (NPSA), the Welsh Government (WG) leads this vital role in identifying any significant safety risks and concerns and since April 2014 has been developing Patient Safety Solutions at a national level for issue to the NHS in Wales.  Guidance on the development of NHS Wales Patient Safety Solutions is available below:

In some cases, additional information is provided to support the management of risks associated with an Alert or Notice. In February 2021, Welsh Government sent this letter to support PSA008 Nasogastric tube misplacement - Welsh Government letter

NHS organisations in Wales are required to submit reports of compliance against each patient safety solution issued by the former NPSA and the Welsh Government.

Documents produced by the NPSA can be accessed via the following external link.

A National Patient Safety Alert (PSA13 June 2021) has been issued to NHS Wales around ligature and ligature point risk assessment tools and policies.

In keeping with guidance around publishing information of this nature, this alert is not available in the public domain.
